Product Description :
Resin bonded anchor system: Epoxy resin adhesive; Two-component, cartridge type; Type IV, Grade 3, Class A, B and C; Approved for use with threaded anchor rods of 3/8, 1/2, 5/8 and 3/4-inch diameters.
Product Restriction :
Ultrabond HS200 shall not be used for overhead applications or where the concrete temperature will exceed 125F degrees. Approved for continued listing 1-1-12. Threaded anchor rods, nuts and washers are by others and Approval per Standard Spec 1-06.1 is required.

Code Description :
A - Visually verify the resin bonding adhesive delivered is the product that was originally submitted by the Contractor and approved from the QPL. Note 'Restrictions' when present. B - Acceptance of threaded rods, nuts, and washers shall be by satisfactory Manufacturer's Certificate of Compliance per WSDOT Standard Specification 1-06.3 for each delivery of material to the project site. Supporting mill tests or documents may also be required with this document. If the Contract contains a Buy America clause, the PEO is responsible for acquiring a Certificate of Material Origin from the Contractor.
